I've attached the Elora-Bethany JNAC which was completed last Spring (2007). The focus was on our 1/2 Minister for Children and Youth, so it doesn't include anything related to my position. Still, I think it might be helpful to others as a model.

Please find attached a session outline for collecting information in preparation for JNAC questionnaires to be built. This format utilizes small groups and was most successful at Alma UC. (The document link.)    Should anyone decide to use this format, I would be happy to come and do a small workshop for small group facilitators. Enjoy!
